I agree, on behalf of my above-noted team, including but not limited to all players, coaches, family members and other spectators, to release Turtle Club Baseball and Softball, LaSalle Titans Baseball and all of their respective affiliates, officers, directors, convenors, coaches, umpires, sponsors, and other officials and volunteers, from any and all liability whatsoever for any injury, loss and/or damage whatsoever arising out of participation in the tournament and/or travel to and from the tournament.
I further agree to pay the tournament fee immediately. Payment of tournament fees are to be made by E-Transfer to [email protected]. Please include your division and team name in the e-transfer message section (e.g. 12U LaSalle Titans). Alternatively, if e-transfer is not possible, a cheque made payable to "LaSalle Titans Baseball" may be mailed to LaSalle Titans Baseball, P.O. Box 37004, 6020 Malden Road, LaSalle, Ontario N9H 1S0.
I acknowledge and agree that payment of tournament fees is non-refundable. We will only consider refund requests if we can fill a team's previously assigned spot with another team.
I further acknowledge and agree that a spot in the tournament is not guaranteed until the club receives payment and confirms, via email, that a spot has been assigned to my team.
NOTE: A roster is not required to be submitted, but birth certificates may be required for verification at any time.
